Please follow this checklist to help ensure a quick and smooth billing process.

Before Your Appointment:

  1. Contact your insurance provider for information about your plan’s specific requirements for your benefits.
  2. Confirm that Crittenton Hospital Medical Center is a contracted provider of your planned services.
  3. Obtain a referral, if required by your insurance company (Discuss this with your primary physician).

At Check-In:

  1. Bring a form of identification and your health insurance cards.
  2. If you are a returning patient, please let us know if your personal or insurance information has changed recently.

After Your Visit: 

  1. Respond promptly to requests from your insurance provider to ensure a smooth billing process.
  2. If you have an out-of-pocket balance due on a bill, please make timely payments.
  3. Contact us if you have any questions about a bill or if you cannot pay your balance in full and need to make other arrangements.
