Patients Rights and Responsibilities

At Crittenton Hospital Medical Center, the patient is our primary concern. If you have any concerns about patient care and safety, you are encouraged to contact Crittenton's hospital leadership at 248-652-5000 and the Operator will direct your call.  You may also contact the Customer Service Comment Line at 248-652-5606

If the concerns cannot be resolved, hospital leadership will advise you to contact The Joint Commission.  Such requests should be addressed to:

Rights:

It is the philosophy of CHMC that patients have a fundamental right to be treated with respect, compassion and courtesy in a safe environment, which protects their personal well being in a private and confidential manner.

The statement that follows is a formal expression of our responsibilities to patients at Crittenton. Please do not hesitate to discuss any concerns regarding your hospitalization or any aspect of your stay at the hospital with members of the medical staff, nursing staff, patient relations or with the hospital administration.

As a patient at Crittenton, you have a right to:

    • Access necessary and appropriate medical care regardless of your race, creed, color, national origin, religion, gender, age, handicap or source of payment.
    • Be treated in an ethical manner, with consideration and respect for your personal dignity, cultural values and beliefs.
    • Be treated in a secure environment, with privacy, confidentiality and the ability to communicate effectively with your caregivers, family and friends.
    • Be fully informed of, and participate in, decisions about all aspects of your care. You may refuse treatment to the extent permitted by law and hospital policy, and have the right to be informed of any of the medical consequences of any such refusal.
    • Have the coordination, facilitation and involvement of your family/guardian in medical treatment decisions, continuity of care, continuing education, and support for normal development of neonates, children and adolescents.
    • Be free from mental and physical abuse, and free from chemical and physical restraint except as ordered by a physician and/or in accordance with applicable hospital policy.
    • Assistance in formulating Advance Directives and in having your directives honored within the limits of the law and Crittenton’s capabilities.
    • Have your pain believed, assessed and managed.
    • Have your comfort and dignity guide all aspects of end of life care.
    • Consult with the Ethics Committee and participate personally or through your representative in any discussion of ethical issues regarding your care.
    • Access to Pastoral Care and other spiritual services upon request.
    • Access to protective services.
    • Ask about and be informed of business relationships that may influence treatment and care.
    • Consent to or decline to participate in research or experimental medical procedures. Any such procedures recommended as a part of medical care will be fully explained. Refusal of such treatment will not in any way jeopardize continued medical care.
    • Receive reasonable attention to complaints and grievances, when communicated to members of the staff.
    • Receive explanation of bills, regardless of sources of payment, and information on financial assistance.
    • Treatment and care in accordance with applicable state and federal law and in compliance with applicable regulatory rules.

 
Responsibilities:

Just as we have responsibilities to you as a patient, you have responsibilities to your hospital and health care providers. You are responsible for:

  • Being honest about everything that relates to you as a patient. Failure to provide us with a complete medical history and your health status habits may seriously impact your prospects of recovery. This includes honesty regarding use of prescribed, over the counter, illicit and herbal drugs.
  • Informing CHMC when you do not understand any aspect about your care and treatment or any lack of understanding of what you are expected to do.
  • Following the recommendations of your physician. Discuss with your physician any dissatisfaction with your treatment plan.
  • Informing your physician if your health status changes in any way.
  • Being considerate of others and seeing that your visitors are considerate of the rights of other patients, of hospital personnel and hospital property. Observing and following the rules, regulations and policies of the hospital.
  • Assuming responsibility for your actions if you refuse treatment or do not follow your physician’s advice.
  • Providing the hospital with accurate information regarding medical insurance coverage.
