Do you or someone you love have diabetes, pre-diabetes, or gestational diabetes. Our experienced, compassionate diabetes educators will show you exactly what you need to do to take control of diabetes and continue to live a long and healthy life.
Diabetes self-management education is the cornerstone of diabetes care. It provides you with the best possible support, education, and resources for your diabetes and will enhance and improve your wellbeing. Diabetes self-management education is important soon after you are diagnosed, and then as your personal needs and goals periodically change.
Our program is customized to meet your individualized needs. We provide you with the very latest information on managing your diabetes. Topics include:
• What is diabetes
• Diabetes treatment options
• Blood glucose monitoring
• Medication management
• Meal planning
• Complications of high and low blood glucose
• Avoiding long term complications
• Dining out and restaurant guides
• Foot care
• Exercise and physical activity
• Starting insulin treatment
Our program includes both group and individual instruction delivered by our diabetes experts. We will work with your primary care physician to provide you with the best possible care customized to meet your needs. Many insurance companies cover diabetes self-management education and medical nutrition therapy.
DIABETES SELF-MANAGEMENT EDUCATION
Crittenton has been recognized by the American Diabetes Association and the Michigan Department of Community Health for Quality Self-Management Education. Comprehensive Diabetes Self-Management Education is available on an ongoing basis. A registered dietitian, registered nurse, exercise physiologist, and others assist you in developing the knowledge you need to successfully manage your diabetes. Your insurance may cover part or all of these services. Please call 248-652-5660 for information and registration.
MEDICAL NUTRITION THERAPY
A registered diewtitian with specialized knowledge in diabetes will meet with you and your family to evaluate your nutritional needs and provide individualized meal planning, instruction, and motivation to improve your diabetes or nutritional health. DIABETES SUPPORT GROUP
Attend this free monthly program for people with diabetes and their family members. Current topics related to diabetes and its management are discussed. Meetings are held at Crittenton, North Building Conference Rooms 2A/B. Call 248-652-5660 for information and registration.
